The registration price does not include GST as the conference is considered a ‘religious service’. Should you require a tax invoice for your records, please photocopy this registration form prior to returning it to us.

I understand and accept that Townsville City Church/ EPIC youth may collect information about me for the purpose of providing and offering promotional material. I consent to my personal details being used for the promotion of future Townsville City Church/ EPIC youth events and products, via post, phone, email & sms. Townsville City Church/ EPIC youth events will be captured in photographs, video and audio. Townsville City Church/ EPIC youth reserves the right to use this material for promotional purposes. I also understand that every reasonable precaution will be taken to ensure the protection of myself. I hereby release Townsville City Church/ EPIC youth, staff and volunteers are released of liability in the event of any injury, accident or misfortune, damage or loss that may occur to myself and/or my property while present at their Event within the Event premises. 

 

If the participant is under the age of 18, parents must read and accept the following to process registration. 

 

I give permission for  Epic Youth, authorized staff and volunteers to obtain emergency medical, hospital or ambulance assistance at any time they consider necessary. I understand that every effort will be made for myself to be notified before instituting such procedures. I acknowledge that I will be liable for any medical/hospital/ambulance expense incurred in the treatment for my child. I also understand that while every reasonable precaution will be taken to ensure the protection of my child, I hereby release and hold harmless EPIC youth, authorized staff and volunteers from any and all liability in the event of any injury, accident or misfortune, damage or loss that may occur to the child and their property while present at the Camp within authorised premises. 

 

By signing the application I give permission for the above child to participate in activities they may choose while attending the event. I agree and understand that Epic youth reserves the right to exercise its discretion to refuse to register any child upon medical and/or other grounds, without providing a detailed reason for so doing.
